Parliamentary paper shoulder numbers and banner of statutory compliance
District health boards (DHBs) and some multi-parent health Crown Entity subsidiaries are required to affix shoulder numbers and banners of statutory compliance to the covers of their Annual Reports and Statements of Intent/Statements of Performance Expectations, as these documents are designated 'Parliamentary Papers'.

Parliamentary Papers are published under the authority of the House of Representatives, and Annual Reports and Statements of Intent/Statements of Performance Expectations must conform to requirements for Parliamentary Papers set by the Office of the Clerk. Under current requirements, Parliamentary Papers must be produced in A4 size and include the appropriate shoulder number in the top right-hand corner of the cover and title page.
Shoulder numbers are issued by the Office of the Clerk (Bills Office). The title page should include the formal name of the Crown Entity and the financial year to which the Annual Report or Statement of Intent/Statement of Performance Expectations relates.
DHBs are reminded that the document provided for tabling can be:
- an SOI incorporating the SPE,
- an Annual Report incorporating the SPE, or
- the (draft) Annual Plan incorporating the SPE.
Parliamentary papers:
• must have their shoulder number printed on the top right-hand corner of the cover/title page. The number remains constant for the same document from year to year (for example, a reporting entity’s annual report, statement of intent, strategic intentions, and statement of performance expectations will always carry the shoulder number assigned to the entity, unless the entity merges with another bigger organisation with its own shoulder number, or it is disestablished);
• should either be printed or photocopied, double-sided, in colour or black and white. They should be A4 size and stapled or perfect bound, not wire-bound or plastic-bound.
• should carry a banner identifying the statutory obligation under which they are being presented clearly labelled on the cover page as follows:
For DHBs that are producing an Annual Plan incorporating the SOI/SPE the banner should read:
Presented to the House of Representatives pursuant to sections 149 and 149(L) of the Crown Entities Act 2004.
and title should read:
2021/22 Annual Plan incorporating the 2020/21-2023/24 Statement of Intent and 2021/22 Statement of Performance Expectations
For DHBs that are producing an Annual Report incorporating the SOI/SPE the banner should read:
Presented to the House of Representatives pursuant to sections 150, 149, and 149(L) of the Crown Entities Act 2004.
and title should read:
2021/22 Annual Report incorporating the 2020/21-2023/24 Statement of Intent and 2021/22 Statement of Performance Expectations
An SOI has not been requested this year by the Minister, however if a DHB has produced a new SOI the following needs to be included:
For those DHBs that are producing a standalone SOI that incorporates the SPE the banner should read:
Presented to the House of Representatives pursuant to sections 149 and 149(L) of the Crown Entities Act 2004.
and title should read:
2021/22-2024/25 Statement of Intent incorporating the 2021/22 Statement of Performance Expectations
